Description
What we provide
The team works with adults aged over 18 and offers an advice and signposting service for you and your carer, whether or not you are eligible for support from the council.
If you are eligible for support from your council, you will work with an assessor to complete your assessment, identify your personal budget and create a support plan highlighting the outcomes you wish to achieve. This support plan will then be shared with the Care Navigation team.
Support we offer
• have local knowledge of groups, organisations, suppliers and services to give you a more personal service
• have knowledge and experience of direct payments, community support and voluntary organisations
• support you to develop a personalised package of support to enhance your quality of life
• build support packages which are new and innovative and allow you to fulfil your potential whilst still receiving the support you need
• work with people who need support to access health and social care.
This includes signposting and information for carers help people who are eligible for support from the council to decide how they would like their support to be delivered. This could be through a brokered service or a direct payment to fund their tailored package or a combination of both. The Care Navigation team will then help you and your carer to find the support identified in your support plan to help you maintain and maximise your independence, choice and control regarding your support.
