Description

Under the government-funded Supporting People scheme, Age UK Sheffield provides short term support (for up to six months) for people aged 55 or over with housing-related needs, living in South East Sheffield.

You may be experiencing difficulty in maintaining independence in your own home, or your lifestyle may be putting your current accommodation at risk.

There are many reasons why people might need this kind of support, for example, being at risk of losing their home due to rent arrears, a history of homelessness, lacking the skills to live independently in the community, or because a trauma, illness or isolation is preventing them from maintaining an independent lifestyle. Following a referral a Support Planner from our service will visit, and agree with you a programme of support tailored to meet your individual needs.

The aim is to enable a person to develop the skills necessary to remain living independently in their home. Once a support plan has been agreed, a support worker will be allocated to work alongside the person to implement the plan. The support plan is reviewed at regular intervals by the Support Planner.

The type of support we provide will typically include some of the following:

• Finding and moving into a new property
• Obtaining furniture
• Home safety check
• Making a full benefit check and applying for relevant benefits
• Budgeting skills and debt management
• Accessing other agencies and organisations providing primary health care
• Making sure essential utilities are connected
• Promoting and encouraging life skills such as cooking, shopping and cleaning
• Preventing social isolation
• Support following a crisis
