If you have been diagnosed with a long term health condition, Kirklees Health Trainers are on hand to help you to learn how to manage your health better.
We can use motivational techniques and positive lifestyle changes to help you to:
• Identify your health needs
• Recognise the triggers that set off your condition
• Care for yourself better and achieve your goals
• Make changes to your life to make you healthier and happier
• Help identify barriers that might prevent you from making healthier choices and look at ways to overcome them by listening, empathising and sharing information
What we provide
Health Trainers work with people who have a long term health condition A Health Trainer will:
• support someone who has been diagnosed with a long term health condition to learn how to identify things that can trigger their illness
• reinforce the behaviour that someone wants to change to improve their health, this could be to achieve more exercise or eat a better diet
• set goals and support an individual to plan how to achieve the goals
• provide information and advice about a long term health condition
• Signpost to other services
• motivate and build confidence to manage health better
• Support a person to manage their condition using a self care approach
Health Trainers also provide personalised information helping people and their carers to become experts in their health needs, supporting them to make changes to become healthier by giving them the tools and knowledge they will need and by working in partnership with primary and secondary care, community services, social care and voluntary services.
Who it is for
The service is for people 18 years and over with long term health conditions. Some of the conditions Health Trainers can help people to self care are:
• COPD (Chronic Obstructive Pulmonary Disease)
• Dementia or memory loss
• Diabetes
• Heart Disease
• Chronic Pain
• Neurological conditions like Parkinson’s disease, MS or Epilepsy
• Mental wellbeing or depression
• Cancer
• Asthma
• Stroke
• Weight management
When it takes place
When the referrals are received. The client can have up to six hour long one to one sessions with a Health Trainer.
