Danny’s Dream provide suitably qualified and experienced Personal Assistants who can help with the provision of a range of health and social care needs either in the home, out and about in the community or, by becoming an exclusive member of the Danny’s Dream “Club” in Walker Street, Hull. We act as the “agent” to ensure that all the responsibility for recruiting, training, organising and paying Personal Assistants and all other regulatory and compliance matters are fully taken care of.
Once a referral has been received we will arrange meetings to ensure service user / family needs and aspirations are fully reflected in an individually created Danny’s Dream support plan. With consent, this process is supported by information taken from other relevant assessments such as Individual Health Care Plans, Epilepsy Plans, Behaviour Management Plans and Occupational Therapy Plans. A full risk assessment is undertaken and this serves to support safe working practices that support all concerned. Once all needs have been fully considered and catered for, a service contract can then be agreed upon and signed. A small team of Personal Assistants will be carefully matched to meet individual service user needs and introductions will be arranged before service delivery begins.
All care plans are viewed as “live” documents and are added to and amended regularly as needs and aspirations change. Over time, additional Personal Assistants may be introduced and this will be achieved by ‘shadowing’ existing shifts. Service users and those involved in their care will be fully involved in this process and prior consent to shadow shifts will always be obtained.
