Description
The Carer's emergency alert card is a laminated card the size of a credit card, which will identify you as a Carer if you have an accident. The scheme is free for anyone that lives within West Sussex and is run by Chichester Careline with support from West Sussex County Council.
The scheme was developed because Carer's were concerned about what would happen to the person they cared for if they were out alone and had an accident. They wanted something that would give them peace of mind and provide help at any time of the day and night.
You can register with the scheme by completing an enrolment form.
You have 3 ways to do this:
1. Complete our online enrolment form and submit it online now.
2. Download the PDF 'Carer's emergency alert card enrolment form'. Complete it in your own time and return it to the address at the bottom of the form.
3. Telephone or e-mail us at Chichester Careline where we will send you a copy of the enrolment form for you to complete and return in your own time.
Once we have received the enrolment form, a confirmation letter and a new Carer's emergency alert card will be sent to you.
The card will have our telephone number on it and your unique reference number. The unique number is linked to your details here at Chichester Carline. Therefore if something should happen, we will then phone your emergency contacts to put into action the procedures you have agreed with us.
The scheme is confidential. Your card will not have any personal details on it, so the identities of you and the person you are caring for are protected. The only way to identify you is by the reference number on the card.
