Description

A single route to information, advice and services including referrals for social care and some health care services. 

Our vision:

We want to provide a service which is easy to use so that people can get the information and advice they need to stay as independent and safe as possible. Our intention is to give people the information they need when they need it the most, while putting them in touch with the appropriate services delivered by the council and partners. 

 The aim of the service is to:

• Safeguard vulnerable adults and children

• Provide an information hub for citizens and partners ensuring support choice and control   is given to Kirklees citizens with a social care or health issue

• Provide easy access to prevention services 

 Social Care:

• Eligibility screening function

• Onward referral for social care assessment

• Safeguarding safe haven and point of contact for Deprivation of Liberty

• Provision of information relating to adult social care

• Point of contact for Asylum Seekers

• Point of contact to support those experiencing domestic violence

• Undertaking of the councils duty under Section 12, section 17 and Section 47 of the Children’s Act

.  Providing non-traditional options for customers who have a presenting social care need

• Referral for Accessible Homes Team

• Extra-care Housing

• Referral for Assistive Technology
