Snowflake Care Ltd aim to deliver a thorough process of ‘active support” working on the premises that we will promote and develop a wide range of life skills with the residents.
We strive to reduce isolation and be active in the community including participation in leisure activities, exercise, faith groups and volunteering. Needs and wishes of each individual will be taken into consideration.
Our model of care is person-centred and based on the recovery model so that each individual is supported to their full potential. There will be a clear structure to community meetings for everyone to share new ideas and lead groups to create confidence and trust.
We will support each individual to attain their goals however small this may be e.g. from a small trip to the coffee shop; going for a meal out or supporting your football team; enrolling for a college course or even finding a job. We will do this by

Ø Liaising with local services together with you by focusing on your strengths and aspirations.
Snowflake Care Ltd aims to empower people to grow, learn, develop and fulfil their own aspirations and dreams, and to achieve the best outcomes possible.
Our staff are highly trained to support you to manage your medication adherence by supervision of medication at the required times e.g. setting reminders and exploring methods to help you do this.
We also offer support to order and collect prescriptions.
Our services are truly person centred. Snowflake Care Ltd is committed to working in partnership
with other agencies such as employment services, community nurses, psychologists, advocates and
allied health professionals etc. We recognise that interdisciplinary communication is paramount for
the best possible outcomes. Snowflake Care Ltd will ensure that residents, carers, relatives, professionals and advocates in care plan development, care plan review and ensuring residents' needs and aspirations are fully documented in the care plan.
Safety
The home is designed for provide a safe and comfortable environment. Snowflake aim to provide an atmosphere where residents experience a positive and honest atmosphere. Residents are able to bring in their own possessions with them.
Staff at Snowflake will ensure the premises are kept clean, hygienic and free from infection. Residents will be protected from all sorts of abuse and any form of abuse will be reported to the
local authority. Residents will be supported to make a complaint if they wish to do so through the accessible channels.
Privacy
Residents will be helped by staff to furnish and equip their rooms using their own décor and
style. Staff will support residents to choose their choice of activity, leisure and entertainment.
Privacy will be provided when residents need time to use their own telephone, opening and
reading mails and communication with friends and relatives or health professionals.
Rooms have locks to provide privacy and service users to be able to have time to be alone when
they wish to.
Dignity
Each resident will be treated as a valued individual. Respect will be given to each individual's religious, ethnic and cultural needs.
Activities within the home will be guided by the choice and interest of the residents.
Personal Care
Snowflake Care Ltd staff will strive to work in partnership with appropriate multi-disciplinary teams involved in each individual's care to provide best possible care and meeting the needs of each
resident.
Staff will liaise with residents, relatives, carers, GP's and professionals to implement and
regularly provide update for plan of care.
A Keyworker is allocated to each resident and will promote individual and group pastimes, encouragement to be involved in the activity of their choice e.g. handicrafts, coffee mornings and
outings to local places of interest.
