We support service users with a range of needs and essential to this is our multi agency working practises. We have links to community multi-disciplinary team and also work alongside drug and alcohol agency /support services, OT’s , social organisations and many other services which support , promote and encourage our service users to ,live a full, meaningful rewarding , independent life, recognising personalisation, choice and control, empowerment, inclusion, fulfilment, dignity, respect, equality and diversity.
We work closely with landlords to secure housing. We support many of our service users who have come from long term residential and hospital settings back into independent living. We support service users to complete all housing benefits if they require help and we continue to support and work alongside the service users and the landlord to help the individual maintain their tenancy, report any faults or problems with their flats and help with any tenancy or neighbour issues that may arise.
Contracts are individual, and invoiced on an hourly basis which reflect the support hours required, we assess in partnership with specialist placement teams, learning disability and assessment care management team, the East Sussex transition service, neighbourhood support teams, strategy, commissioning and supply management along with the individual themselves and any family members involved with the co-ordination of their care.
