Our outreach service provides one of our qualified and trained staff members to give 1-to-1 personal support to you or your relative. Unlike domiciliary support we take individuals to go out into the community. We can also provide support in your own home.We offer a no-obligation, free-of-charge initial meeting to discuss your requirements. Please send us a Request for Service to initiate this.
Emerald Care started in 2005 and have specialised experience in supporting people in the community and in our residential homes. We are registered with CQC – the government body who regulated care. Our team of highly experienced and qualified Support Workers have provided a quality and needs led one to one service for adults of all ages with autism for over six years. We will help each individual to set their own programme of activities – balanced to meet the agreed aims of support.
We will provide that much needed space and a break for parents who act as carers for their own son or daughter.
Active support caters for a wide range of individuals, with a range of issues on the autistic spectrum including those with challenging behaviours Support is offered in full hour blocks between four and eight hours per day and can be arranged for any number of days per week. Support will be given either on a one to one or two to one basis as required.
Our team work with individuals who:
Receive local authority funding
or
Receive Independent Living Fund (ILF) payments
or
Have private funding
The Activities below are only intended as an example of some of the activities that our team undertake. We pride ourselves in being able to offer a wide variety of services away from people’s homes.
Physical Activities - Swimming, gym, badminton, 10 pin bowling, walking…
Training - Self care, road safety, accessing public transport.
Social and leisure - Local clubs and groups, day centres, pub visits, theatre, cinema, sporting events…
All the activities we provide support with will contain elements of social skills and independence training. All our staff have gained extensive experience of working with people who have Autism, through working with individuals in our residential settings before moving on to the outreach services in the community. They have all received formal training working with people who challenge and those on the Autistic Spectrum. The staff are highly professional and self motivated, enabling our users to receive a top quality service that will meet their individual varied needs. We strive to provide flexible community support tailored to individual needs. We undertake an assessment, which acknowledges each individual carer’s views and we also liaise with other agencies.
With the benefit of one to one or two to one support we are able to focus on skill development and the promotion of independence by integrating into the local community to access all the facilities available. We aim to provide a balanced timetable of activities appropriate to the person and set agreed goals, which we will record in their personalised care plan.
